Self-Payers: If you have not received an ESU Branch TLab Scholarship, you are a self-payer. There is a mandatory $250 deposit for self-payers due with this application. You have the option of printing this completed form and mailing it in with a check or paying online via PayPal. You will not need to create a PayPal account in order to use your credit card.

Cancellation Policy:
If you are unable to attend the program, you must notify the ESU immediately in writing at auhl@esuus.org and when applicable cc the ESU Branch that awarded a TLab Branch Scholarship. Fees already paid to the Globe will be returned according to the Globe's cancellation policy, as specified in their materials. ESU Branch TLab Scholarship Recipients will be personally liable for additional TLab fees such as stipend and/or airfare if disbursed or paid prior to departure in the event of a withdrawal.
If the State Department of the United States announces a "no travel" policy or the Globe cancels a program, fees paid to the institution will be returned according to the individual school's cancellation policy, as specified in their materials.
International Airfare:
It is highly recommended to:
1. Not book international airfare until you have received your email acceptance to the 2024 TSTP program directly from The Globe.
2. to book only a changeable/refundable flight.
ESU TLab Branch Scholars should Not book international airfare until given permission by your ESU Branch.

In order to receive contact hours for completing this program, I agree to:
- Implement and disseminate the information I have learned within my school community
- Provide to the ESU evidence of implementation and dissemination signed by a principal or department chair
- Share with the ESU the modes/methods of that implementation (lesson/unit plans; classroom activities, exercises, and assessments; worksheets) within 4 months of completion.
