Membership Recognition Award

Please use this form to nominate an individual or couple that your Branch believes to be worthy of recognition. They will be honored & acknowledged nationally at the opening ceremony of the 2024 ESU Annual Conference in Cleveland, Ohio.  Please submit your responses by June 7, 2024.  Nominate a worthy candidate and please complete one form per person or couple.
